About the position

The Registered Nurse Case Manager (RNCM) at Sanctuary Hospice is responsible for planning, organizing, and directing hospice care for patients and their families. This role involves utilizing the nursing process to assess, plan, implement, and evaluate care while collaborating with interdisciplinary team members. The RNCM is dedicated to providing compassionate and high-quality end-of-life care, ensuring patient comfort and quality of life.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with the Clinical Director to assess the eligibility and suitability of clients for hospice services.
• Provide direct care and support to patients, ensuring their comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques to ease pain and bring solace to patients.
• Educate and empower patients and families to participate in creating personalized care plans.
• Share the responsibility of call duties, demonstrating commitment to being available when it matters most.

Requirements

• Current unencumbered license as a Registered Nurse (RN).
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• Experience in hospice care is a plus.
• Proficient in typing and various computer programs, including EMR systems.
• Current CPR certification as required by state.
