About the position

The RN Case Manager I PRN Days at Shelby Medical Center is responsible for facilitating patient care through effective resource coordination, ensuring optimal health outcomes, and appropriate utilization of healthcare resources. This role emphasizes compliance with medical necessity standards, transition management, and care coordination to promote timely discharges and prevent avoidable readmissions. The RN Case Manager will work collaboratively with healthcare teams to educate patients and families about care plans and ensure that care is delivered at the appropriate level based on individual patient needs.

Responsibilities

• Facilitate care along a continuum through effective resource coordination.
• Ensure care is provided at the appropriate level based on medical necessity.
• Assess patient transition needs to promote timely throughput and safe discharge.
• Lead and facilitate multi-disciplinary patient care conferences.
• Manage concurrent disputes and make appropriate referrals to other departments.
• Communicate with patients and families about the plan of care.
• Collaborate with physicians, office staff, and ancillary departments.
• Document timely and concisely in the Case Management system.
• Maintain accurate patient demographic and insurance information.
• Identify and report over and underutilization of resources.

Requirements

• Graduate of an accredited school of nursing.
• Active RN license for state(s) covered.
• 2 years of acute hospital patient care experience.

Nice-to-haves

• Academic degree in nursing (bachelor's or master's).
• Acute hospital case management experience.
• Accredited Case Manager (ACM) certification.
