About the position

The RN Case Manager for Long Term Care is responsible for coordinating patient care within a specific territory in Chelsea, Massachusetts. This role involves working under the supervision of a Clinical Manager or Hospice House Manager to ensure that patients receive comprehensive care in accordance with professional standards. The RN will assess patient needs, develop care plans, and collaborate with an interdisciplinary team to provide optimal support to patients and their families.

Responsibilities

  • Coordinate all aspects of patient care in accordance with professional standards.
  • Identify patient and family needs and provide supportive care as per physician's orders.
  • Participate in ongoing assessments and case conferences to coordinate care plans.
  • Supervise and direct others in planning and implementing patient care.
  • Maintain regular communication with supervisors and attending physicians regarding patient status.
  • Provide 24/7 coverage to patients and families, including on-call responsibilities.
  • Assess, plan, implement, and evaluate care for hospice patients in various settings.
  • Document patient records accurately and ensure compliance with care standards.
  • Provide clinical supervision to ancillary staff and volunteers.
  • Support families in bereavement and provide follow-up care.

Requirements

  • Registered Nurse, currently licensed in Massachusetts in good standing.
  • Associate's degree in nursing required; Bachelor's degree preferred.
  • Minimum three years of varied work experience as a professional nurse, with one year in an acute setting preferred.
  • Demonstrated ability to assess and respond to patient and family needs.
  • Skilled in nursing practice and able to cope with family emotional stress.
  • Understanding of hospice philosophy and principles.
  • Ability to work effectively within an interdisciplinary team.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Knowledge of community resources for patient support.
  • Certification in case management or related field.
