About the position
The RN Case Manager position within the Pediatric Complex Care Collaborative (PC3) program at Johns Hopkins Children's Center focuses on improving the lives of complex patients and their families through coordinated care. The role emphasizes collaboration with a multi-disciplinary team to streamline patient experiences and connect families with necessary resources, ensuring care continuity and support for patients at home whenever possible.
Responsibilities
- Coordinate and streamline patient and family experiences across hospital settings and the care continuum.
- Serve as the primary point of contact for patients and families within the PC3 program.
- Employ nursing processes to gather objective and subjective data about patients.
- Respond to patient calls and MyChart messages, triaging urgent symptoms as needed.
- Provide patient education for both acute and chronic medical conditions.
Requirements
- Completion of an accredited RN nursing program; BSN preferred.
- Minimum one year of Pediatrics RN experience required; ideal candidate will have 5 years' RN experience.
- Valid Maryland Nursing License required.
- Preferred experience in case management, discharge planning, or utilization review.
- Accredited Case Manager - Registered Nurse (ACM - RN) / Certified Case Manager (CCM) preferred.
