About the position

The RN Case Manager Registry position at Sinai Chicago involves providing specialized wound care treatment to patients in various outpatient healthcare settings. The role focuses on assessing, evaluating, and developing individualized wound care plans, collaborating with healthcare teams, and educating patients and their families on effective wound care techniques.

Responsibilities

  • Provide specialized wound care treatment to patients in various outpatient healthcare settings
  • Assess and evaluate patients' wounds, including pressure ulcers, surgical wounds, and diabetic ulcers
  • Develop and implement individualized wound care plans based on patient needs and medical history with the help of the provider
  • Administer wound dressings, medications, and treatments as prescribed by healthcare providers
  • Monitor and document patients' progress in healing wounds and adjust treatment plans as necessary
  • Collaborate with interdisciplinary healthcare team members to coordinate patient care and ensure optimal outcomes
  • Educate patients and their families on wound care techniques, prevention strategies, and self-care measures

Requirements

  • Valid nursing license in the state of practice
  • Certification in wound care (WCC) preferable
  • Proven experience in providing wound care treatment to patients
  • Knowledge of various wound care products, dressings, and treatment modalities
  • Familiarity with electronic medical records (EMR) systems for documentation purposes
  • Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
  • Ability to work independently and make sound clinical judgments in a fast-paced environment
  • Flexibility to adapt to changing patient needs and priorities
  • Associate Degree in Nursing (ADN) and two (2) or more years' experience as a clinical RN
  • Two (2) years in an acute care setting
  • Current RN license in the state of practice to be maintained throughout the duration of employment in the position
  • Bachelor of Science in Nursing (BSN) preferred
  • Medical Surgical, Home Health or Wound Care Certification preferred
  • Wound care experience preferred
