About the position

The RN Case Manager (Senior) position at the University of New Mexico's Center for Development and Disability involves providing comprehensive case management services for medically fragile individuals and their families across New Mexico. The role requires travel primarily in the Southeast region and involves working with an interdisciplinary healthcare team to develop and implement health care plans. The RN Case Manager will conduct assessments, facilitate care transitions, and ensure continuity of care for clients.

Responsibilities

  • Provide comprehensive case management services for medically fragile individuals and their families.
  • Conduct phone visits, telehealth visits, and home or community visits.
  • Develop comprehensive health care plans in collaboration with individuals, families, and the interdisciplinary health care team.
  • Complete annual level of care assessments and participate in interdisciplinary team meetings.
  • Assist individuals and families in navigating and accessing community-based health care services.
  • Travel to rural and frontier areas as needed for client visits.

Requirements

  • Bachelor's degree in Nursing or a related clinical field.
  • At least 1 year of experience directly related to the duties and responsibilities specified.
  • State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure.
  • CPR Certification.

Nice-to-haves

  • Experience in pediatric and/or community health nursing.
  • Experience with home visiting and accessing community resources.
  • Experience working with children/families who are medically fragile or have special health care needs.
  • Bilingual in Spanish and English.
