About the position

A Plus Home Care, Inc. is seeking a dedicated and adaptable Registered Nurse for the Nurse Supervisor position. This part-time role offers flexible hours ranging from 15 to 30 hours per week, allowing for a customizable schedule and the option to work remotely. The Nurse Supervisor will be responsible for overseeing field staff, conducting client assessments, and creating care plans to ensure quality home care services.

Responsibilities

• Conduct assessments of client needs
• Oversight and supervision of field staff
• Meeting with new clients to open case files and complete new open paperwork
• Meeting with existing clients on a consistent basis to review cares, services and paperwork
• Creation of client Care Plans to direct services
• Training of field staff on specific client cares
• Charting of progress notes based upon scheduled visits
• Collaboration and care coordination with office staff and case managers
• Review of field staff to assess competency and knowledge
• Be a resource for field staff regarding client questions and concerns

Requirements

• Current RN license in the state of WI
• Current CPR certification
• Home care experience of at least 1 year
• Independence/time management skills
• Reliability and ability to work autonomously
• Team player attitude
• Positive attitude and professionalism
• Organizational skills
