About The Position

Bristol Hospice is seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to provide exceptional, patient-centered end-of-life care. The role involves creating and implementing care plans, supporting patients and their families, and ensuring the highest level of compassion, respect, and dignity in care delivery. The RN Case Manager will work closely with patients, conducting assessments and administering treatments as prescribed, while also documenting care and health information accurately.

Requirements

• An RN license in good standing.
• 1 year of experience in hospice or home health preferred.
• A valid driver's license.
• Current CPR certification.
• Excellent observation, verbal, and written communication skills.

Nice To Haves

• Experience in Microsoft Excel.

Responsibilities

• Create and initiate care plans through assessing the history of current and previous illnesses.
• Spend quality one-on-one time with patients and their families to support everyday needs.
• Care for patients by providing initial and ongoing assessments of patient and family needs.
• Record important health information including temperature, pulse, respiration, and blood pressure.
• Administer medications and treatments as prescribed by the physician.
• Provide health care instructions to the patient as appropriate.
• Record symptom management changes and outcomes.
• Complete, maintain, and submit accurate and relevant clinical notes regarding the patients condition and care given.
• Perform additional duties as assigned.
