About the position

The RN Case Manager is responsible for coordinating patient care and ensuring the appropriate utilization of healthcare resources. This role involves evaluating patient needs, facilitating transitions to the correct level of care, and collaborating with various departments to enhance patient outcomes. The position aims to provide cost-effective and clinically appropriate care while supporting discharge processes when necessary.

Responsibilities

• Coordinate care and progression status for patients.
• Facilitate movement to the most appropriate level of care.
• Evaluate patient needs proactively and provide discharge support as needed.
• Integrate with other departments and work closely with ED and admitting physicians.
• Implement procedures consistent with the Sinai Health System Utilization Review Plan.

Requirements

• Associate degree in Nursing required; Bachelor's degree in Nursing preferred.
• Three years of healthcare case management experience.
• Registered nurse in the state of Illinois.
