About the position

The Registered Nurse Case Manager at InnovAge is responsible for coordinating acute and post-acute care for admitted participants, ensuring optimal health outcomes and preventing hospital readmissions. This role involves collaboration with healthcare providers and facility staff to develop and implement individualized care plans, manage discharge planning, and facilitate communication among all parties involved in the care process.

Responsibilities

• Perform daily coordination of acute and post-acute care with facility staffing.
• Assist providers and facility staff in managing InnovAge admitted participants.
• Review medical necessity and level of care appropriateness in collaboration with the InnovAge IDT.
• Coordinate post-facility discharge planning and support utilization review and improvement activities.
• Assess, develop, plan, and evaluate care provided to participants while admitted to hospital settings.
• Collaborate with providers and interdisciplinary health care team members to develop and document individualized plans of care.
• Participate in daily IDT meetings and formulate Plans of Care for InnovAge PACE program participants.
• Recommend alternative levels of care and ensure compliance with regulations.
• Communicate discharge plans to providers, patients, family/caregivers, and community agencies.
• Document all necessary information and maintain participant medical records.

Requirements

• Associate degree in nursing
• Current State-issued Registered Nurses License
• 3 years coordinating care and discharge planning (preferred)
• 3 years health care experience with emphasis in geriatrics (preferred)
• Bachelor's degree in nursing (preferred)
• Bilingual (preferred)
• Certification as a Gerontological Nurse (preferred)
