About the position

As a Registered Nurse (RN) Case Manager at CommonSpirit Health Mountain Region, you will play a crucial role in advocating for patients and ensuring they receive the best possible care throughout their healthcare journey. This position involves collaborating with interdisciplinary healthcare partners, facilities, and families to facilitate care coordination and connect patients with necessary resources. You will utilize your clinical expertise and independent judgment to assess and reassess patient needs, ensuring that medical appropriateness criteria are met for their care status and level.

Responsibilities

• Advocate for patients and collaborate with interdisciplinary healthcare partners, facilities, and families.
• Utilize clinical expertise and independent judgment in assessing and reassessing patient needs.
• Facilitate care coordination and utilization management for patients.
• Ensure medical appropriateness criteria are met for status and level of care.
• Connect patients to resources that help them maintain and build on positive outcomes.

Requirements

• Nursing Diploma or Associates Degree in Nursing required; BSN preferred.
• RN License in Applicable State of Employment required.
• 1 year of nursing or case management experience required.
• Experience working with EMR preferred.
• Working knowledge of regulatory requirements and accreditation standards strongly preferred.
• BLS from the American Heart Association required.

Nice-to-haves
