About the position
The RN Case Manager is responsible for coordinating the continuum of care activities for assigned patients, ensuring optimal utilization of resources, service delivery, and compliance with medical regimes. Responsibilities
- Coordinate continuum of care activities for assigned patients.

Requirements
- Current RN licensure in the state of practice.
- At least one year of Case Management experience preferred.
- Current CPR certification if applicable.
- TB questionnaire, PPD, or chest x-ray if applicable.
- Current Health certificate as per contract or state regulation.
- Must meet all federal, state, and local requirements.
- Must be at least 18 years of age.
