About the position

Kaiser is seeking a Per Diem RN Case Manager for their Vandever Medical Office location in San Diego, CA. This role involves care coordination for patients transitioning from inpatient care to acute rehabilitation, focusing on discharge planning and ensuring compliance with regulatory standards. The position offers a set schedule of 30-40 hours per week, primarily Monday through Friday, with no weekend work required.

Responsibilities

• Coordinate care for patients discharging from inpatient to acute rehab.
• Manage discharge planning processes.
• Utilize principles of utilization review and case management.
• Ensure compliance with regulatory requirements and accreditation standards.
• Communicate effectively with patients and healthcare teams.

Requirements

• Inpatient Medicare RN License
• Bachelor of Science in Nursing
• Mid-level Case Management experience
• Working knowledge of regulatory requirements (TJC, Medicare, Medi-Cal)
• Strong communication skills
• Critical thinking and problem-solving skills
