About The Position
The RN Case Manager is responsible for coordinating and facilitating patient care throughout the continuum of care, ensuring that services provided are patient-focused, high quality, efficient, and cost-effective. This role involves direct interaction with patients and their families to assess needs and develop individualized care plans in collaboration with physicians. The RN Case Manager also monitors resource use and length of stay, communicates with the Utilization Management team, and ensures effective communication among all members of the healthcare team.
