About the position

The RN Case Manager is responsible for overseeing the delivery of coordinated care for patients, managing clinical care activities, and ensuring effective communication among healthcare team members. This role involves both fieldwork and administrative duties, requiring strong clinical knowledge and supervisory skills.

Responsibilities

• Oversee the delivery of coordinated care for patients.
• Manage day-to-day clinical care activities within the assigned area.
• Provide accurate documentation to ensure patient reauthorization.
• Conduct in-home RN scheduled visits and attend physician appointments.
• Assist in implementing and amending the Plan of Care (POC).
• Orient new staff and provide training and follow-up support.
• Submit nursing documentation timely per company policy.
• Complete assigned chart reviews and counsel staff on documentation issues.
• Establish collaborative relationships with patients, physicians, and payers.
• Assist the authorization team with timely information for patient reauthorization.
• Participate in quality assurance committees and interdepartmental projects.
• Demonstrate knowledge of Payor policies and procedures.
• Assist with state survey preparation and implementation.

Requirements

• Bachelor's degree in a related field.
• Current Registered Nurse (RN) license to practice in relevant states.
• Current Basic Cardiac Life Support certification.
• At least one year of case management experience.
• At least one year of staff scheduling experience.

Nice-to-haves

• Three or more years of experience as a Registered Nurse.
• Nurse management or supervisory experience.
• Home health or Department of Labor (DOL) experience.
