About the position

The RN Case Manager is responsible for enhancing the quality of patient management in a home health setting, specifically for Medicare patients. This role involves overseeing the patient care cycle, including intake, assessment, planning, implementation, monitoring, and evaluation of care. The case manager will advocate for patients, facilitate communication among the medical team, and ensure patient satisfaction while promoting cost-effectiveness.

Responsibilities

  • Coordinate and provide safe, timely, effective, efficient, equitable, and client-centered care.
  • Facilitate multiple care aspects including case coordination and information sharing.
  • Help patients make informed decisions by advocating for their clinical status and treatment options.
  • Develop effective working relationships and cooperate with the medical team throughout the case management process.
  • Interact with patients to track their progress and ensure satisfaction.
  • Record case information, complete necessary forms accurately, and produce statistical reports.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.

Requirements

  • Valid RN License
  • Valid CPR Certification
  • Proven working experience in case management in home health
  • Excellent knowledge of case management principles and healthcare management
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem-solving skills and ability to multi-task
  • Compassionate with teamwork skills

Nice-to-haves

  • Experience with Riversoft program knowledge
