About the position
Join our compassionate team as a Hospice Registered Nurse Case Manager! This role involves planning, organizing, and delivering in-home hospice care, focusing on patient comfort and family support. The ideal candidate will have community health experience and will guide families and patients through end-of-life care with dignity and empathy.
Responsibilities
- Conduct comprehensive assessments of patients and families.
- Develop and update individualized care plans, administering medications and treatments as needed.
- Provide nursing care, health education, and emotional support to patients and families.
- Coordinate care with physicians and interdisciplinary teams.
- Participate in on-call duties and ensure seamless care continuity.
Requirements
- Valid RN license.
- Experience in hospice or community health care.
- Strong communication and case management skills.
- Ability to work independently and compassionately in patient homes.
