About the position

The RN Case Manager/Care Coordinator is responsible for managing patient care and coordinating services to ensure optimal health outcomes. This role involves working closely with patients, families, and healthcare teams to develop and implement care plans, monitor progress, and facilitate communication among all parties involved in patient care.

Responsibilities

• Manage patient care and coordinate services to ensure optimal health outcomes.
• Develop and implement individualized care plans for patients.
• Monitor patient progress and adjust care plans as necessary.
• Facilitate communication between patients, families, and healthcare teams.
• Provide education and support to patients and their families regarding care plans and health management.

Requirements

• Current RN license in New Mexico.
• BLS certification from an approved provider (American Heart Association, American Health and Safety, or American Red Cross).
• Experience in case management or care coordination.
