About the position

The RN Case Manager role involves managing a caseload of patients in a field-based setting, ensuring they receive appropriate care through home visits and regular assessments. The position requires a valid RN license and entails significant travel to patient residences, with a focus on developing and implementing care plans. The role is full-time, with a structured schedule that includes both fieldwork and weekly office meetings.

Responsibilities

• Manage a caseload of 15-20 patients.
• Conduct daily visits to patients' residences, averaging 5 visits per day.
• Ensure appropriate care plans are developed and implemented for each patient.
• Attend weekly team meetings at the office.
• Comfortably enter patient homes for assessments and care delivery.

Requirements

• Valid RN License (Required)
• 1 year of RN experience (Required)
• 1 year of case management experience (Preferred)
• Ability to relocate to Glendora, CA before starting work (Required)
• Willingness to travel 75% of the time (Required)
