About the position

The Home Health Case Manager is responsible for enhancing the quality of patient management, maximizing satisfaction, and promoting cost-effectiveness in a home health setting. This role involves overseeing the patient care cycle, including intake, assessment, planning, implementation, monitoring, and evaluation of care actions required to meet clients' health and human services needs.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Facilitate multiple care aspects including case coordination and information sharing.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Develop effective working relationships and cooperate with the medical team throughout the entire case management process.
  • Interact with patients to track their progress and ensure satisfaction.
  • Record case information, complete all necessary forms accurately, and produce statistical reports.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.

Requirements

  • Current RN License
  • CPR Certification
  • Proven working experience in case management in home health
  • Excellent knowledge of case management principles and healthcare management
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem-solving skills and ability to multi-task
  • Compassionate with teamwork skills

Nice-to-haves

  • Experience in home health case management
  • Knowledge of Riversoft program
