About The Position
The RN Case Manager is responsible for ensuring timely and effective discharge planning for patients within 24 hours of admission. This role involves assessing the patient's physical and mental condition, psychosocial concerns, and support systems, while utilizing critical thinking skills to facilitate ongoing discharge planning throughout the patient's stay. The RN Case Manager communicates effectively with the healthcare team, patients, and families to ensure a smooth transition when the patient is medically ready for discharge.
