About the position

The RN Case Manager is responsible for ensuring timely and effective discharge planning for patients within 24 hours of admission. This role involves assessing the patient's physical and mental condition, psychosocial concerns, and support systems, while utilizing critical thinking skills to facilitate ongoing discharge planning throughout the patient's stay. The RN Case Manager communicates effectively with the healthcare team, patients, and families to ensure a smooth transition when the patient is medically ready for discharge.

Responsibilities

• Complete a physical, mental, and psychosocial assessment within 24 hours of patient admission.
• Communicate effectively with the multidisciplinary team to facilitate discharge planning.
• Document assessments and interventions in the patient's medical record.
• Assist patients and families in developing coping skills and provide emotional support.
• Discuss financial alternatives and implications with patients and families.
• Participate in and lead interdisciplinary meetings as necessary.
• Serve as a resource for the multidisciplinary team.
• Collaborate with social services to ensure effective discharge planning.
• Act as a liaison between Froedtert South and external referral sources.
• Attend and present in-services and educational programs to enhance professional expertise.

Requirements

• Valid RN license in Wisconsin.
• 1 to 3 years of nursing experience in a healthcare setting.
• Ability to read, write, speak, hear, and comprehend the English language.
