About the position

This position is part of the clinical team. This position is responsible for a variety of care management activities with a focus on ensuring that all patients receive the support they need. This position will identify areas of reeducation needs, will coordinate training needs and serve as a liaison among staff, to help create continuity around the organization as related to patient quality, experience and satisfaction.

Responsibilities

  • Responsible for supervision of clinical staff, including but not limited to hiring, staffing, discipline, training/education and performance evaluation.
  • Observes and provides feedback on the practice of the clinical staff to ensure appropriate patient care, as well as proper implementation of the required programs related to quality measures.
  • Serves as the primary contact for assigned clinical staff (scheduling/changes/updates/coordination of staff).
  • Collaborates with providers and other multidisciplinary team members to provide complete patient care.
  • Serves as the primary contact for patient and/or service complaints, investigates complaints as appropriate and involves other staff (i.e. Risk Management), as needed.
  • Ensure the proper usage of the electronic health record.
  • Provide individual professional advice and support to clinical staff, as needed.
  • Travel to all assigned sites for training and implementation of processes as needed.
  • Participates in the development of new and/or enhancement of existing policies, procedures, and programs, to improve accuracy and quality of patient care.
  • Assists with developing new and/or refining existing health improvement initiatives to best meet the health needs of patients, ensuring program utilizes evidence-based practices and reach targeted population.
  • Works in a direct patient care capacity on a regular basis and filling in for staffing needs.
  • Works in conjunction with the director of nursing, vaccine compliance coordinator, and practice management.
  • Works independently with minimal supervision, working toward accomplishment of established goals.

Requirements

  • Licensed Registered Nurse in the State of Kansas; strongly prefer multi-state Registered Nurse license.
  • Previous experience in a clinic setting preferred.
  • Valid Driver's License required.
