About the position
The RN Clinical Liaison at Gentiva Hospice plays a crucial role in educating healthcare professionals about hospice services and providing compassionate end-of-life solutions. This position is dedicated to ensuring that patients and their families receive the necessary support and care during critical times, emphasizing clinical excellence and dignity.
Responsibilities
- Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
- Complete the admissions process on referred patients in emergency cases and for inpatient hospice admissions as staffing indicates.
- Provide accurate and timely information regarding hospice services to inquiries.
- Receive calls from referral sources, respond timely, and initiate paperwork to begin the intake/referral process.
- Serve as a patient advocate in evaluating/developing a plan of care and obtaining informed consent for hospice services.
- Call on multiple account types including SNFs, ALFs, hospitals, and physicians.
- Offer in-services and education on the benefits of hospice.
Requirements
- RN license in the state(s) of employment.
- Minimum of 2 years of healthcare experience preferred in settings such as hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Compassionate, kind, and empathetic demeanor towards clients, their patients, and families.
- Known and respected within the community.
