About the position
The RN Clinical Liaison at Gentiva Hospice plays a crucial role in educating healthcare professionals about hospice services and providing compassionate end-of-life solutions. This position focuses on ensuring effective intake and referral processes to meet the needs of patients and their families, while advocating for patient care and facilitating the admissions process.
Responsibilities
- Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
- Complete the admissions process on referred patients in emergency cases and for inpatient hospice admissions as staffing indicates.
- Provide accurate and timely information regarding hospice services to inquiries.
- Receive calls from referral sources, respond timely, and initiate paperwork to begin the intake/referral process.
- Serve as a patient advocate in evaluating/developing a plan of care and obtaining informed consent for hospice services.
- Call on multiple account types including SNFs, ALFs, hospitals, and physicians.
- Offer in-services and education on the benefits of hospice.
Requirements
- RN license in the state(s) of employment.
- Minimum of 2 years of healthcare experience preferred in settings such as hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Compassionate, kind, and empathetic demeanor towards clients, their patients, and families.
- Known and respected within the community.
