About the position

The RN Clinical Manager for Home Health at CenterWell is responsible for coordinating and overseeing all direct care patient services provided by clinical personnel. This role focuses on ensuring clinical quality assurance and operational initiatives within home health services. The manager will develop, implement, and analyze clinical operations, manage caregiver assignments, and ensure compliance with regulatory standards while providing leadership and support to clinical staff.

Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for the location managed.
  • Conduct and delegate patient assessments and update care plans as necessary.
  • Manage the assignment of caregivers and oversee the delivery of care to all patients.
  • Receive case referrals and determine home health or hospice needs based on patient information.
  • Ensure patients meet admission criteria and make decisions regarding patient admissions.
  • Instruct and guide clinicians to promote effective performance and quality home care services.
  • Monitor cases for compliance with documentation and regulatory requirements.
  • Coordinate communication between team members, physicians, and caregivers for appropriate care planning.
  • Work with the Branch Director and Finance Department to establish revenue and budget goals for the location.
  • Participate in sales and marketing initiatives for the branch.
  • Supervise all clinical employees and handle employee corrective actions fairly and objectively.
  • Participate in the hiring, training, and development of direct care clinicians.
  • Evaluate staff performance and recommend in-service education programs as needed.
  • Conduct regular staff education based on clinical documentation reviews and feedback.
  • Analyze situations and identify problems using Performance Improvement principles.
  • Review clinical staff documentation for appropriateness of care and compliance with requirements.
  • Responsible for QA/PI activities and data tracking for performance review.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years of experience as a Registered Nurse with at least one year of management experience in home care or hospice.
  • Home health experience is required.
  • Management and people leadership experience is required.

Nice-to-haves

  • OASIS experience preferred.
  • OASIS certification (HCS-O, COQS, and/or COS-C) preferred.
  • Homecare Homebase (HCHB) experience preferred.
  • CMS PDGM billing knowledge and/or experience preferred.
