About the position

The RN Clinical Manager at CenterWell Home Health is responsible for coordinating and overseeing all direct care patient services provided by clinical personnel at the Lincolnton, NC location. This role involves developing and implementing clinical operations, managing caregiver assignments, ensuring compliance with regulatory standards, and participating in performance improvement activities. The manager also plays a key role in staff training and development, as well as maintaining communication with team members and external stakeholders to ensure quality patient care.

Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for the location.
  • Conduct and delegate patient assessments and update care plans.
  • Manage the assignment of caregivers and ensure delivery of care to all patients.
  • Review patient information to determine home health or hospice needs and make admission decisions.
  • Instruct and guide clinicians to promote effective performance and quality home care services.
  • Assist clinicians in establishing therapeutic goals and developing patient Plans of Care (POC).
  • Monitor cases for compliance with documentation and regulatory requirements.
  • Coordinate communication between team members, physicians, and caregivers.
  • Work with the Branch Director and Finance Department to establish revenue and budget goals.
  • Participate in sales and marketing initiatives.
  • Supervise all clinical employees and handle employee corrective actions fairly and objectively.
  • Participate in the hiring, training, and development of direct care clinicians.
  • Assess staff education needs and conduct regular staff education as needed.
  • Analyze situations and identify problems using Performance Improvement principles.
  • Review clinical staff documentation for appropriateness of care and compliance.
  • Oversee QA/PI activities and work with Utilization Review staff on performance data tracking.
  • Provide direct patient care in emergencies and act as Branch Director in their absence.
  • Interpret Company standards and ensure compliance with regulatory authorities.
  • Maintain relationships with referral/community sources and participate in professional organizations.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.

Nice-to-haves

  • Experience with Homecare Homebase (HCHB) software and OASIS.
  • Strong clinical background in home care operations and quality assurance.
