About the position

Serenity Hospice, founded over 10 years ago, is dedicated to providing exceptional hospice care, particularly to veterans. Initially, the organization focused solely on veteran patients, but has since expanded its services to the Portland, Salem, and Eugene communities. As a member of our team, you will assume critical responsibilities, including triaging after-hours patient and family phone calls. This involves assessing the need for services such as symptom management and care plan updates, and delegating nursing visits to appropriate staff members. You will also be responsible for gathering comprehensive data on the physical, psychological, social, and spiritual factors that may affect the needs of patients and their families, ensuring that interventions by other members of the interdisciplinary team are initiated as necessary. Effective communication is key in this role. You will initiate contact with attending physicians, interdisciplinary staff members, and other agencies to coordinate optimal care for our patients. Additionally, maintaining accurate clinical records is essential, which includes the timely preparation of care plans and clinical progress notes. This position is not only about providing care but also about ensuring that every aspect of the patient's experience is managed with professionalism and empathy.

Responsibilities

  • Assume responsibility for triage of after-hours patient and family phone calls.
  • Determine the need for services including assessments, symptom management, and care plan updates.
  • Delegate nursing visits to appropriate staff members.
  • Obtain data on physical, psychological, social, and spiritual factors impacting patient and family's needs.
  • Initiate intervention by other members of the interdisciplinary team as appropriate.
  • Initiate communication with the attending physician and other interdisciplinary staff members for optimal care coordination.
  • Maintain an accurate clinical record through timely preparation of care plans and clinical progress notes.

Requirements

  • Graduate from an accredited registered nursing program.
  • Minimum of two years of varied work experience as a professional nurse.
  • Demonstrated skill in nursing practices and clinical judgments.
