About the position

The RN Complex Care Manager Outpatient role at PeaceHealth is a part-time position focused on overseeing and evaluating the coordination of integrated population management for high-risk patients. This role involves developing care management programs, collaborating with healthcare professionals, and ensuring effective care coordination to improve clinical and financial outcomes. The position requires a blend of clinical expertise, leadership, and interpersonal skills to manage a panel of complex patients and support caregivers in delivering quality care.

Responsibilities

  • Oversee and evaluate coordination of integrated population management across selected populations.
  • Develop care management programs addressing a wide scope of health care needs and conditions.
  • Identify, coordinate, and case manage a panel of clinically high-risk patients.
  • Provide oversight for caregivers managing patients at moderate or low risk.
  • Accountable for development and continuous evaluation of population management programs and care coordination services.
  • Collaborate with system and regional leadership to optimize processes and metrics.
  • Risk-stratify patients according to protocols and assess patient data.
  • Work with patients and families to create and implement care plans that meet identified needs.
  • Adjust care plans based on patient activation and risk levels through ongoing monitoring.
  • Delegate and oversee care management of lower-risk patients and routine chronic disease management tasks.
  • Track, evaluate, and provide oversight for transitions of care, ensuring effective communication between clinics and care facilities.
  • Demonstrate leadership, mentorship, and teamwork within dedicated care teams.

Requirements

  • Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred; Master's Degree in Nursing preferred.
  • Minimum of 3 years clinical nursing experience in an ambulatory or clinic setting required.
  • Registered Nurse in state of practice required.
  • Basic Life Support certification required upon hire; Advanced Cardiac Life Support required within 6 months; Care Manager Certified preferred within 2 years.
  • Skill and proficiency in applying highly technical principles, concepts, and techniques required.
  • Basic knowledge of statistics required.
  • Interpersonal skills to obtain and interpret information appropriate to patients' needs required.
  • Demonstrated ability to promote positive interpersonal relationships and patient/family-centered work environment required.
  • Demonstrated ability to delegate and supervise the performance of tasks required.
  • Demonstrated ability to provide training, mentorship, and development to clinic staff required.
  • Basic computer skills including MS Office applications required.
  • Effective prioritization skills required.

Nice-to-haves

  • Care Management or Care Coordination experience preferred.
  • Utilization management/quality management experience preferred.
