About the position
NortheastArc-Clinical Services is seeking Registered Nurses (RNs) for part-time and full-time positions in Worcester County. The role involves providing Continuous Care Nursing for medically fragile pediatric and adult patients in their homes, schools, or community settings. Nurses will work one-on-one with patients, ensuring they receive the necessary medical care to avoid hospitalization. The agency specializes in a wide range of medical conditions, including tube feeding, tracheostomy care, and other complex nursing needs.
Responsibilities
- Provide one-on-one nursing care to patients in home and community settings.
- Administer medications and treatments as prescribed.
- Monitor and assess patients' health status and report changes to the healthcare team.
- Collaborate with families and interdisciplinary teams to develop and implement care plans.
- Educate patients and families on managing medical conditions and care needs.
- Maintain accurate and timely documentation of patient care activities.
Requirements
- Valid RN license in the state of Massachusetts.
- Proficiency in nursing skills and critical thinking.
- Experience in home care or ICU-level care is preferred but not mandatory.
- Strong communication skills to interact effectively with patients, families, and healthcare teams.
- Compassionate and empathetic approach to patient care.
- Ability to adapt to diverse patient needs and environments.
Nice-to-haves
- Experience with tube feeding and tracheostomy care.
- Background in pediatrics or working with medically fragile patients.
