About the position

The RN Coordinator for Ambulatory Operations at UT Southwestern is responsible for coordinating multidisciplinary clinical care for patients, ensuring compliance with ACO requirements. This role involves leading a team of clinical staff, monitoring patient quality metrics, and facilitating effective communication and workflows to enhance patient care.

Responsibilities

  • Function as team lead for a group of clinical staff assigned to specific patients to ensure efficient and effective patient care coordination.
  • Ensure all patient quality metrics are completed by staff during clinic visits to meet ACO requirements.
  • Assign clinical duties as needed to ensure effective workflows and patient care requirements are met.
  • Work with supervisor to ensure staff has been oriented and trained for assigned duties.
  • Monitor patient registries for patients assigned to respective POD.
  • Coordinate follow-up and assign duties to clinical staff for patients in POD registries to close care gaps.
  • Monitor patient telephone requests and MyChart in-baskets for timely response to patient communications.
  • Coordinate team assignments to effectively respond to patient messages.
  • Perform other duties as assigned.

Requirements

  • Strong communication skills and the ability to facilitate team-based care.
  • BLS (Basic Life Support) Certification by the American Heart Association or American Red Cross upon hire.
  • Registered Nurse (RN) license from the Board of Nurse Examiners for the State of Texas upon hire.

Nice-to-haves

  • 2 years of ambulatory clinical nursing experience.
