About the position

The RN Coordinator for AdventHealth Hospital at Home (AHHaH) plays a crucial role in ensuring that patients receive safe and high-quality hospital-level clinical care in the comfort of their own homes. This position is designed for individuals who are passionate about providing excellent consumer experiences and are committed to maintaining the highest standards of patient care. The RN Coordinator will work closely with patients, their families, social workers, nurses, providers, and the interdisciplinary team to identify patients who meet the inclusion criteria for the AHHaH program. This role involves coordinating all aspects of patient care transfer from the acute care hospital to the patient's home, ensuring that acute care level services are maintained while also focusing on patient satisfaction and safety. In this position, the RN Coordinator will communicate daily with the interdisciplinary team during multidisciplinary rounds, responding to any changes in the plan of care by coordinating in-home resources, durable medical equipment (DME), or supplies. The RN Coordinator is responsible for evaluating patients' post-AHHaH needs, developing transition of care plans, and implementing post-acute care discharge plans. Additionally, this role serves as an educational resource for patients, family members, providers, and clinical staff, ensuring that everyone involved in the patient's care is informed and engaged. The RN Coordinator will adhere to departmental and system goals, objectives, policies, and procedures, as well as comply with CMS Conditions of Participation for discharge planning and care coordination. This position actively participates in outstanding customer service and is responsible for maintaining respectful relationships with all stakeholders involved in patient care. Responsibilities

  • Ensure safe and quality hospital-level clinical care for AdventHealth Hospital at Home patients.
  • Identify patients meeting the inclusion criteria for the AHHaH program in collaboration with the interdisciplinary team.
  • Coordinate all aspects of patient care transfer from the acute care hospital to the patient's home.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Respond to plan of care changes by coordinating in-home resources, DME, or supplies.
  • Conduct patient evaluations of post-AHHaH needs and develop transition of care plans.
  • Serve as an educational resource for patients, family members, providers, and clinical staff.
  • Adhere to departmental and system goals, objectives, policies, and procedures, ensuring quality patient care and regulatory compliance.
  • Participate in outstanding customer service and maintain respectful relationships with all stakeholders.
  • Review patient information to ensure at-home services are coordinated and complete initial AHHaH assessments prior to transfer.

Requirements

  • Associate in Nursing or above.
  • Minimum of 2 years of Acute Care Nursing experience.
  • Minimum of 1 year of Acute Care Case Management experience.
  • Ability to work and communicate with people of diverse backgrounds.
  • Registered Nurse licensure in Florida.
  • Basic Life Support certification.
  • Valid Florida driver's license.

Nice-to-haves

  • Bachelor's degree in nursing.
  • Master's degree in nursing or a health-related field.
