About The Position

The RN, Developmental Disabilities (IDD) Nurse Case Manager is responsible for providing direct and indirect patient care, as well as coordinating services for individuals with developmental disabilities. This role involves case management, care coordination, and ensuring positive health outcomes for patients. The nurse will work closely with the IDD care team, manage referrals, and oversee medication administration and treatment compliance.

Requirements

  • Current California RN license.
  • Current CPR certification.
  • Current ACLS certification.
  • Minimum of three years of experience in direct patient care as an RN.
  • Minimum of one year of experience in developmental disability nursing preferred.
  • Demonstrated proficiency with all tasks on the RN IDD Nurse Case Manager competency list.
  • Knowledge of the nursing process and critical thinking abilities.
  • Knowledge of principles of nurse case management.
  • Knowledge of electronic medical records and tracking systems.
  • Fluency in English and Spanish preferred.
  • Strong interpersonal skills and ability to be sensitive with diverse populations.
  • Experience with Microsoft Office applications including Outlook, Word, Excel, and PowerPoint.
  • Strong organizational skills with the ability to prioritize projects and manage multiple tasks.

Nice To Haves

  • Fluency in Spanish (speak, read, and write) preferred.

Responsibilities

  • Assist with meeting the needs of individuals with disabilities, including assessment, education, referrals, and follow-up care.
  • Provide nursing care including administering medications and treatments in compliance with medical providers and standing orders.
  • Conduct solo and shared nursing office visits for patients with IDD for specialized care/management.
  • Act as the point person for triage for ARFPSHN and other specialized homes.
  • Perform medication reconciliation and chart review for ARFPSHN and other specialized homes weekly.
  • Interpret information from patients' health records and document all clinical care in the electronic medical record.
  • Track referrals to the IDD team and schedule appointments for primary care and consults.
  • Assist clinicians and care team in accessing care and coordinating referrals to outside community services.
  • Provide general care coordination of case managed patients, including patient triage, phone call-backs, and prescription refills.
  • Schedule and track IDD dental visits in coordination with the dental scheduler and program manager.
  • Collaborate with dental staff to ensure positive outcomes for IDD patients' dental visits.
  • Educate and counsel patients about disease management and medication adherence.
  • Participate in ongoing training and quality improvement projects.
  • Act as a resource for clinical staff, service partners, and patients/caregivers.
