About the position

The Field Care Manager Nurse 2 plays a crucial role in assessing and evaluating the needs of members to help them achieve and maintain optimal wellness. This position involves coordinating care for members residing in nursing facilities and employing various strategies to manage their physical, environmental, and psycho-social health issues. The role requires independent decision-making and the ability to navigate complex situations while ensuring that patients progress towards their desired health outcomes.

Responsibilities

  • Assess and evaluate member's needs to achieve optimal wellness.
  • Coordinate care for members residing in nursing facilities.
  • Create member care and service plans for long-term care facilities.
  • Monitor patient care through continuous assessments and evaluations.
  • Identify and resolve barriers to effective care.
  • Make independent decisions regarding work methods in ambiguous situations.

Requirements

  • Unrestricted Registered Nurse License in Indiana.
  • 2 years of experience in in-home case/care management.
  • Experience working with the adult population.
  • Knowledge of community health and social service agencies.
  • Ability to use electronic medical records and various software applications.
  • Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel.
  • Exceptional communication and interpersonal skills.
  • Ability to work with minimal supervision and a flexible schedule.

Nice-to-haves

  • BSN degree.
  • 3-5 years of in-home assessment and care coordination experience.
  • Experience with health promotion, coaching, and wellness.
  • Previous managed care experience.
  • Certification in Case Management.
  • Motivational Interviewing Certification.
