About the position

The Field Care Manager Nurse 2 plays a crucial role in assessing and evaluating the needs of members to help them achieve and maintain optimal wellness. This position involves guiding members and their families to appropriate resources for care and well-being, coordinating care for those in nursing facilities, and employing various strategies to manage health issues. The role requires independent decision-making and effective communication to ensure members progress towards their health goals.

Responsibilities

  • Assess and evaluate member's needs to achieve optimal wellness.
  • Guide members and families towards appropriate resources for care.
  • Coordinate care for members residing in nursing facilities.
  • Create member care and service plans for long-term care facilities.
  • Monitor patient care through continuous assessments and evaluations.
  • Identify and resolve barriers to effective care.
  • Make independent decisions regarding work methods in ambiguous situations.

Requirements

  • Unrestricted Registered Nurse License in Indiana with 2 years of in-home case/care management experience.
  • Experience working with the adult population.
  • Knowledge of community health and social service agencies.
  • Ability to use electronic information applications and electronic medical records.
  • Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel.
  • Exceptional communication and interpersonal skills.
  • Ability to work with minimal supervision and a flexible schedule.

Nice-to-haves

  • BSN
  • Experience in long-term care or nursing home settings.
  • 3-5 years of in-home assessment and care coordination experience.
  • Experience with health promotion, coaching, and wellness.
  • Previous managed care experience.
  • Certification in Case Management.
  • Motivational Interviewing Certification or knowledge.
