About The Position
The RN Field Case Manager at Hope At Home Health Care is responsible for providing skilled nursing care to patients in their homes, ensuring adherence to physician orders and the established plan of care. This part-time position offers flexibility in scheduling and the opportunity to work in various locations within the Tri-County area of Michigan. The ideal candidate will have experience in home health care, particularly in areas such as ICU, PICC line management, wound care, and IV infusion.
