About the position

The Health and Social Services Coordinator at UnitedHealthcare plays a crucial role in the Integrated Care Model by coordinating long-term care needs for patients in the local community. This position involves assessing, planning, and implementing individualized care strategies while advocating for patients to ensure they receive the best possible care. The role requires a significant amount of fieldwork, with the coordinator spending about 80% of their time visiting members in their homes or long-term care facilities, supported by a dynamic team.

Responsibilities

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage the care plan throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team.

Requirements

  • Current, unrestricted licensure as a Registered Nurse in the state of Massachusetts
  • 2+ years of clinical experience
  • 1+ years of experience with MS Office including working with Word, Excel, and Outlook
  • Ability to travel in assigned region to visit members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

Nice-to-haves

  • BSN, Master's Degree or Higher in Clinical Field
  • CCM Certification
  • 1+ years of community case management experience coordinating care for individuals with complex needs
  • Experience working in team-based care
  • Background in managed care.
