About the position

The RN Field Supervisor is responsible for monitoring and assessing the delivery of home health field services, supervising Home Health Aides (HHA), and providing case management support. This role involves clinical intervention, development of care plans, and follow-up as needed, all under general supervision. The position requires bilingual Spanish skills to effectively communicate with patients and caregivers.

Responsibilities

• Perform initial and ongoing supervisory field visits to patients' homes for assessment and training support.
• Convey significant changes in the patient's condition and report interventions to the clinical manager.
• Collaborate with the primary care physician and clinical team to gather patient medical history and medication information.
• Supervise the initial home patient assessment to ensure compliance with VNS guidelines.
• Identify training needs of the home health aide and document electronically.
• Deliver the start of care packet and review it with the patient/caregiver during the initial visit.
• Educate and review the paraprofessional plan of care with the Home Health Aide.
• Participate in quality assurance and education programs as requested by management.
• Engage in special projects and perform other duties as assigned.

Requirements

• License and current registration to practice as a Registered Professional Nurse in the State of New York required.
• CPR/BLS Certification required.
• Associate's Degree in Nursing from an approved program required.
• Bachelor's Degree in Nursing from an approved program preferred.
• Minimum of two years in a clinical background as a Registered Nurse required.
• Home care experience in a licensed home care service agency or CHHA settings preferred.
