About the position
Communities of Care is seeking a compassionate and skilled Full Time or Part Time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide home care services to clients in the Twin Cities area. This role involves working closely with medically fragile pediatric and adult clients, ensuring their comfort and health through dedicated nursing care. New graduates are welcome to apply, and there is no vaccine requirement for this position.
Responsibilities
- Administer nursing care to clients
- Help maintain patients' treatment plans
- Monitor and report changes in patient symptoms or behavior
- Maintain accurate patient medical records
- Provide advice and support to patients and their family members
Requirements
- RN or LPN Minnesota License
- BLS Certification
- ACLS Certification
- 11+ years of experience preferred
- Strong problem solving and critical thinking skills
Nice-to-haves
- Experience with tracheostomies and ventilators
- Ability to build rapport with patients
