About the position

The Registered Nurse (RN) at East Park Retirement Community provides direct and compassionate care to patients in a healthcare or assisted living facility. This role involves assessing patient needs, creating and implementing care plans, administering medications, and collaborating with healthcare professionals to ensure optimal patient care. The RN is expected to demonstrate clinical expertise, strong communication skills, and a commitment to patient safety and well-being.

Responsibilities

  • Conduct thorough assessments of patient or resident health status, including physical, mental, and emotional health.
  • Develop individualized care plans based on patient needs, collaborating with the interdisciplinary team to ensure comprehensive care.
  • Administer medications, treatments, and therapies according to physician orders, facility policies, and regulatory guidelines.
  • Monitor patient responses to treatments and adjust care plans as needed, reporting any changes in condition to the appropriate healthcare team members.
  • Perform clinical procedures such as wound care, IV therapy, catheter care, and respiratory treatments in accordance with facility protocols and best practices.
  • Provide patient and family education on medications, treatments, and lifestyle adjustments to promote health and wellness.
  • Assist with diagnostic testing, monitor vital signs, and ensure proper documentation of patient care activities.
  • Safely administer medications and treatments, following the five rights of medication administration.
  • Ensure accurate documentation of medication administration and promptly report any adverse reactions or errors.
  • Monitor and manage medications for patients with chronic conditions or complex medication regimens.
  • Maintain accurate, thorough, and timely documentation of all patient care activities in the electronic health record (EHR).
  • Ensure compliance with federal, state, and local regulations, as well as facility policies related to patient care and documentation.
  • Participate in quality assurance initiatives to maintain and improve the standard of care.
  • Communicate effectively with patients, families, and the interdisciplinary care team to ensure a coordinated and holistic approach to care.
  • Act as an advocate for patients and their families, ensuring their needs and preferences are met.
  • Participate in care conferences and team meetings to discuss patient progress, care plans, and any necessary adjustments.
  • Respond promptly to medical emergencies, providing life-saving interventions such as CPR and airway management.
  • Advocate for the rights and needs of patients, ensuring they receive respectful, compassionate, and person-centered care.
  • Provide emotional support to patients and families, helping them navigate difficult health situations and decisions.

Requirements

  • Graduate of an accredited school of nursing (Associates or Bachelors degree in Nursing).
  • Active and unrestricted Registered Nurse (RN) license.
  • Strong clinical and assessment skills, with the ability to develop and implement effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
  • Ability to work effectively as part of an interdisciplinary team and collaborate with other healthcare professionals.
  • Proficiency in electronic health records (EHR) systems and nursing documentation.
  • Knowledge of nursing best practices, medications, and safety protocols.
