About the position

The RN Healthcare Manager at Optum is responsible for supporting patients with high-risk health conditions, helping them navigate the healthcare system. This role involves patient education, advocacy, and collaboration with multidisciplinary care teams to ensure optimal patient outcomes. The Healthcare Manager works in a self-directed environment, performing nursing duties within the scope of their RN license, and contributes to the overall mission of improving healthcare for the community.

Responsibilities

  • Identify patients at high risk in collaboration with providers and clinic staff.
  • Support longitudinal care of patients with chronic conditions through assessments, medication reconciliation, and patient education.
  • Assist in the transition of patients from inpatient to outpatient settings by assessing transitional needs and providing post-discharge support.
  • Coordinate with providers to establish or update patient care plans.
  • Document accurately and timely in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.

Requirements

  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience as an RN.
  • Active and unrestricted RN license in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases such as COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills for electronic medical records.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills and ability to work with multidisciplinary teams.
  • Excellent organizational and prioritization skills.

Nice-to-haves

  • Experience in patient education and motivational interviewing skills.
  • Fluent written and verbal skills in English and Spanish.
