About the position

The RN Healthcare Manager at WellMed is responsible for supporting patients with high-risk health conditions, helping them navigate the healthcare system. This role involves patient education, advocacy, and collaboration with multidisciplinary care teams to ensure optimal patient care and quality of life. The Healthcare Manager operates in a self-directed environment, performing nursing duties within the scope of their RN license.

Responsibilities

  • Identify patients at high risk in collaboration with providers and clinic staff.
  • Support longitudinal care of patients with chronic conditions through assessments, medication reconciliation, and patient education.
  • Assist in the transition of patients from inpatient to outpatient settings by assessing transitional needs and providing necessary education and resources.
  • Coordinate with providers to establish or update patient care plans.
  • Document accurately and timely in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings as required.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.
  • Perform clinical tasks within the scope of practice as needed.
  • Complete all other related duties as assigned.

Requirements

  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills to work efficiently with electronic medical records.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills and ability to work with multidisciplinary teams.
  • Excellent organizational and prioritization skills.

Nice-to-haves

  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting.
  • Fluent written and verbal skills in English and Spanish.
