As a Hospice RN/RN Case Manager, you will play a crucial role in delivering compassionate care to patients at a pivotal time in their lives. You will report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient. Your responsibilities will include acting as the primary liaison between the physician, patient/family, and hospice team, providing direct patient care services, and offering supportive care to the patient's family. You will manage a reasonable caseload while fostering trusting and compassionate relationships with your patients and their families. Additionally, you will empower caregivers by educating them on how to care for their loved ones, collaborate with an interdisciplinary team, evaluate patient and family needs, and assist in the preparation and execution of individualized care plans. You will also be responsible for maintaining all assigned patient clinical records and participating in company quality assessment and performance improvement programs.

Responsibilities

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
  • Maintain primary control and professional management of each patient.
  • Act as the primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Empower patient's caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary group consisting of LPNs, CNAs, and support staff.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.

Requirements

  • Valid RN License required.
  • Experience in hospice or palliative care preferred.

Nice-to-haves

  • Experience in home health care.
