About the position

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their treatment journey. You will work closely with patients, families, and healthcare providers to develop individualized care plans that address both medical and behavioral health needs.

Responsibilities

• Conduct comprehensive diagnostic evaluations to assess patient needs and develop appropriate care plans.
• Provide high-quality patient service by addressing concerns and facilitating access to necessary resources.
• Collaborate with interdisciplinary teams to ensure effective communication and coordination of care across various settings, including primary care and outpatient services.
• Monitor vital signs and other health indicators to evaluate patient progress and adjust care plans as needed.
• Advocate for patients by navigating healthcare systems and connecting them with community resources.
• Deliver education on health management strategies, including medication adherence and lifestyle modifications.
• Assist in emergency medicine situations when necessary, ensuring timely intervention for patients in crisis.
• Document all patient interactions accurately in electronic health records to maintain compliance with regulatory standards.

Requirements

• Current Registered Nurse (RN) license in California
• Experience in case management or a related field is preferred.
• Strong knowledge of behavioral health principles and practices.
• Proficiency in conducting assessments and developing care plans tailored to individual patient needs.
• Excellent communication skills, both verbal and written, with a focus on patient-centered care.
• Ability to work collaboratively within a multidisciplinary team environment.
• Familiarity with outpatient services and emergency medicine protocols is an asset.
• Commitment to providing compassionate, high-quality patient care while maintaining confidentiality.
