About the position
The RN Home Care Intake Coordinator plays a crucial role in assessing patients' home health care needs and coordinating referrals to home care and hospice services. This position is essential for ensuring continuity of care and optimal patient outcomes, working closely with both internal and external healthcare providers.
Responsibilities
- Participate in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
- Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care.
- Provide education and strategies for patients and families.
- Coordinate referrals to home care to promote continuity of care.
- Partner with other VNA clinicians and external providers to execute best patient outcomes.
- Document all changes in patient's condition and update care plans and outcomes.
Requirements
- Valid NJ nursing license
- Bachelor's degree in Nursing preferred
- 2+ years of community health nursing experience
- Excellent verbal and written communication skills
- Excellent interpersonal and organizational skills
- Strong client focus and decision-making skills
- Creative in problem-solving
