About the position

The RN Home Health Care position at American Advantage Home Care, Inc. involves providing and directing home nursing care to homebound senior citizens in the Detroit area. The role requires a compassionate and skilled nurse who can effectively apply the nursing process in accordance with agency policies and procedures. The nurse will be responsible for making initial evaluations, developing and implementing care plans, and ensuring continuity of care by coordinating with other health professionals. This position is crucial in serving diverse populations and promoting patient independence through education and support. The RN will perform a variety of nursing tasks, including venipunctures, infusion therapy, wound care, and other specialized nursing skills as outlined in the treatment plan signed by the physician. Regular evaluations of the client's needs will be conducted, and the nurse will initiate necessary nursing procedures while making referrals to other disciplines as needed. The role also includes teaching and counseling clients and their families about home nursing procedures, disease prevention, and injury management. Additionally, the RN will supervise home health aides and LPNs, ensuring that care plans are communicated effectively and adjusted as necessary. Accurate documentation of client care progress and compliance with agency policies is essential. The nurse will also participate in quality improvement programs and plan for client discharge from services, making this a dynamic and impactful role in the healthcare field.

Responsibilities

  • Provides and directs the provision of home nursing care based on agency policies and procedures.
  • Makes the initial evaluation visit to the client and accepts only clients referred by the client care manager or designee.
  • Demonstrates competency in nursing skills including venipunctures, infusion therapy, wound care, and aseptic technique.
  • Evaluates and regularly reevaluates the nursing needs of the client, initiating and revising the client's plan of care as necessary.
  • Initiates diagnostic, preventive, and rehabilitative nursing procedures appropriate to the client's care and safety.
  • Observes signs and symptoms, reporting to the physician and/or other health professionals as needed.
  • Teaches, supervises, and counsels the client and family regarding home nursing procedures and care needs.
  • Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals.
  • Supervises and teaches home health aides and LPNs as needed, assessing the client's need for unskilled care every two weeks.
  • Develops, prepares, and maintains individualized client care progress records with accuracy and timeliness.
  • Participates in the agency's quality improvement program and evaluates outcomes of care.
  • Plans for client discharge from services.

Requirements

  • Successful completion of Nursing Program including the designation of RN.
  • Licensed to practice professional nursing in the State of Michigan.
  • Minimum of 1 year of home health care nursing experience (preferred).

Nice-to-haves
