About the position

Presbyterian Healthcare Services is seeking a skilled Home Health Case Manager for their Complete Care Program. This role involves managing the case of Home Healthcare patients, ensuring that care delivery aligns with quality and utilization guidelines, and achieving desired clinical outcomes in home or community settings. The Home Health Case Manager is responsible for the completeness, timeliness, and appropriateness of patient care plans, their implementation, and ongoing evaluation, while also serving as a patient advocate and ensuring accurate documentation.

Responsibilities

• Manage case of Home Healthcare patients.
• Ensure care delivery is within quality and utilization guidelines.
• Achieve desired clinical outcomes for disease states in home or community settings.
• Assure completeness, timeliness, and appropriateness of patient care plans.
• Implement and evaluate patient care plans continuously.
• Communicate with multidisciplinary teams, support services, and community resources.
• Advocate for patients and ensure timely, accurate, and complete documentation.

Requirements

• Current RN License in the State of New Mexico or Compact State Nursing License.
• Minimum one year of acute care experience within the last three years.
• Basic typing competency.
• Current driver's license and auto insurance.
• BLS certification required at time of hire.
• ACLS and PALS certifications.
