About the position

The RN Home Health Case Manager at Freedom Home Health Care is responsible for leading a team of nurses to manage care for a group of 35-40 clients. This role involves developing patient care plans using a holistic approach, coordinating care during admissions, transfers, and discharges, and conducting home visits for medication setup, wound care, and patient education. The position emphasizes collaboration with visit nurses to ensure quality care delivery.

Requirements

• CPR Certification
• RN License
• Mid-level Driver's License
• Experience in case management
• Knowledge of home health care plans
• Nursing experience of at least 1 year (preferred)
