About the position

As a Home Health Registered Nurse at CenterWell, you will play a crucial role in providing comprehensive home care for patients managing chronic conditions or recovering from various health issues. Your responsibilities will include case management, skilled nursing visits, and coordinating care with other healthcare professionals to ensure optimal patient outcomes. You will work under the supervision of the Clinical Manager, collaborating with physicians to develop and revise treatment plans based on ongoing assessments.

Responsibilities

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians to develop the initial plan of treatment based on physician's orders and initial patient assessment.
  • Provide hands-on care, management, and evaluation of the care plan, teaching patients in accordance with physician orders.
  • Revise care plans in consultation with physicians based on ongoing assessments and regulatory requirements.
  • Coordinate appropriate care with various healthcare personnel, including therapists and home health aides.
  • Report patient care, condition, and progress to the patient's physician and Clinical Manager continuously.
  • Implement patient care plans in conjunction with patients and families to assist in achieving optimal resolutions of needs/problems.
  • Coordinate, oversee, and supervise the work of Home Health Aides and Personal Care Workers, providing written care instructions that reflect the current plan of care.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing necessary clinical documentation.
  • Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, and care plan updates.

Requirements

  • Diploma, Associate or Bachelor Degree in Nursing
  • Minimum of one year nursing experience preferred
  • Strong med surg, ICU, ER, acute experience
  • Home Health experience a plus
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation.
