About the position
The Home Health Registered Nurse (RN) position is a per diem role focused on providing skilled nursing care to home health patients. The RN will be responsible for admission, case management, and follow-up visits, ensuring that patients receive the necessary care and support in their homes. This role involves collaboration with physicians and the Clinical Manager to develop and revise treatment plans based on patient assessments and ongoing evaluations.
Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Confer with physicians to develop the initial plan of treatment based on physician's orders and initial patient assessment.
- Provide hands-on care, management, and evaluation of the care plan, teaching the patient in accordance with physician orders under Clinical Manager's supervision.
- Revise the care plan in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Requirements
- Current RN license in the state of Florida.
- Experience in home health nursing or a similar field.
- Strong assessment and critical thinking skills.
- Ability to work independently and manage time effectively.
Nice-to-haves
- Certification in Home Health Nursing (CHHN) or similar specialty certification.
- BLS certification.
