About the position

As a Home Health Registered Nurse at CenterWell, you will play a crucial role in providing skilled nursing care to home health patients. This part-time position involves case management, patient assessments, and coordination of care with various healthcare professionals to ensure optimal patient outcomes. You will work under the supervision of the Clinical Manager, developing and revising care plans in collaboration with physicians and providing hands-on care to patients in their homes.

Responsibilities

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians to develop the initial plan of treatment based on physician's orders and initial patient assessment.
  • Provide hands-on care, management, and evaluation of the care plan, teaching patients in accordance with physician orders.
  • Revise care plans in consultation with physicians based on ongoing assessments and as required by policy/regulation.
  • Coordinate appropriate care with various healthcare personnel, including Physical Therapists and Home Health Aides.
  • Report patient care, condition, and progress to the patient's physician and Clinical Manager continuously.
  • Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
  • Coordinate, oversee, and supervise the work of Home Health Aides and Personal Care Workers, providing written personal care instructions.
  • Monitor the appropriate completion of documentation by home health aides as part of supervisory responsibilities.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
  • Prepare appropriate medical documentation on all patients, including case conferences and care plan changes.
  • Prepare visit/shift reports, update patient records, and confer with other healthcare disciplines.

Requirements

  • Diploma, Associate or Bachelor Degree in Nursing.
  • Minimum of one year nursing experience preferred.
  • Strong med surg, ICU, ER, acute experience.
  • Home Health experience a plus.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
